New Patient Center

First Visit

Depending on the complexity of the condition, a new patient encounter will last approximately 45 – 60 minutes and include a history, physical examination, diagnosis, and treatment.

Please bring all related paperwork to the first visit including: completed new patient forms, current insurance card, surgical reports, and X-ray/MRI reports. New patients can complete all required forms prior to their first visit. Also, anyone being seen for a lower extremity condition should also bring or wear shorts, sweat pants or loose clothing.

Scheduling

We realize that your time is valuable and respect your schedule. We make every effort to run on time and do not overbook our patients. We request 24 hour notice if you find it necessary to cancel an appointment.

Insurance

Prior to your first visit we encourage you to verify your insurance benefits to fully understand your coverage. We do check insurance coverage but ultimately it is your responsibility to understand how you may be covered. We will do our best to make sure your insurance coverage (or lack of) does not interfere with your health care needs.

Payment

It is our policy to collect all fees at the time of service. Convenient methods of payment include: cash, check, Visa/Mastercard, and American Express.
